We chose the name Agate Health. Like the stone—formed under great pressure and believed to support healing—we too are multi-faceted and resilient.

Each founding member has personally battled chronic illness. This is deeply personal. Our team has overcome extreme challenges in their individual healthcare journeys, and we consider it a privilege to align ourselves with this crucial cause.

We view patient empowerment and clinical efficiency as interconnected challenges, solved through innovative problem-solving and data-driven insights. By making patients active contributors to their healthcare team, we're advancing patient engagement while ensuring providers stay ahead of the curve.

We are bringing forward a dynamic care model with innovative strategies to equip patients to take a more active role in their healing. Our global network of advisors includes physicians, nurses, administrators, and most importantly, patients.

We're a diverse team with unconventional backgrounds. Founders Naciima Mohamed and Joseph Cohen are an unlikely pair—and that's our greatest strength. Naciima, a 19-year-old Somali-American, who received the coveted Gates Scholarship, left college to lead this vital mission. A former Team USA athlete, she has also led impactful initiatives worldwide, driving change in communities across the globe. Joseph, a 39-year-old disabled Navy veteran, lifelong hacker and Ohio State math graduate. He's a two-time cancer survivor and helped develop the nation's largest Prescription Drug Management Program.
